Community Support Services can help an adult with I/DD participate more fully in everyday community life. For some people, that may mean support with activities, routines, appointments, social participation, or skill-building connected to their plan. For others, it may mean having structured help to stay engaged, safe, and included.

The service should always be tied to the person’s authorized plan of care. A provider should not promise a one-size-fits-all program or describe Community Support Services as a medical service. The purpose is community-based support that reflects the participant’s assessed needs and goals.

What Good Community Support Looks Like

Good support is respectful, practical, and person-centered. It may help the participant practice communication, navigate community settings, maintain routines, participate in meaningful activities, or build confidence in familiar places. The work should be planned, documented, and coordinated with the broader support team.

Why Families Value Community Support

Isolation can become a real barrier for adults with disabilities. Community Support Services can help create structure around inclusion rather than leaving families to figure out everything alone. The best results usually come when the participant’s preferences are taken seriously and the provider communicates consistently with the case manager.

What It Is Not

Community Support Services should not be used as a vague label for every kind of help. It is not a medical clinic, skilled nursing visit, transportation company, or recreational program by itself. Activities and transportation may be involved only when they are appropriate within the authorized service and plan.
