Essential Living Support, LLC provides Community Support Services in Cheyenne, Wyoming for adults with intellectual and developmental disabilities through Wyoming DD Waiver and HCBS services. CSS helps participants practice self-help, social, adaptive, and community participation skills in real community settings, according to the participant’s Individualized Plan of Care.

How CSS Supports Community Participation
Community Support Services are designed to keep participants engaged in their environment, build on previously learned skills, and support appropriate behavior, independence, and personal choice. CSS is a habilitation service, so training on objectives and progress documentation are part of the service.
Progress must be documented and made available each month to the participant, legally authorized representative, and case manager.
Where Community Support Services Happen
Under the Wyoming DD Waiver Service Index, CSS must be scheduled in settings separate from the participant’s residence. It must be furnished in a variety of community settings and cannot be limited only to fixed-site or congregate settings. Transportation is a component of CSS and is included in the provider rate.
How CSS Differs From Community Living Services
Community Living Services focus on residential and daily living support connected to where a participant lives. Community Support Services are non-residential supports scheduled outside the participant’s residence and centered on community access, relationships, adult learning when authorized, and skill reinforcement.
How CSS Differs From Adult Day Services
Community Support Services are a habilitation service and must be provided in varied community settings separate from the participant’s residence. Adult Day Services provide meaningful daytime activities for adults age 18 and older and are not a habilitation service. The participant’s case manager and plan of care team help determine which service fits the IPC.
